Community Health Worker Program

Community Health Workers serve as a bridge between the patient, provider, and social services. They help reduce the cost of health care by linking patients to community resources and educating patients on the proper use of their medical provider versus hospital/emergency department.

  • Marketplace Insurance Applications
  • Medicaid Applications
  • SNAP Benefit Applications
  • Coordinate transportation for medical, dental and behavioral health appointments
  • Lifestyle education for diabetes and hypertension
  • Referrals to social service organizations and help completing any paperwork needed for services
  • Home visits to assess environments to help improve health outcomes
