Community Health Workers serve as a bridge between the patient, provider, and social services. They help reduce the cost of health care by linking patients to community resources and educating patients on the proper use of their medical provider versus hospital/emergency department.

- Health Insurance Marketplace applications
- Medicaid expansion applications
- Coordinate transportation to and from medical, dental, or behavioral health appointments
- Health patients work toward health goals
- Make referrals to social service organizations and help with the paperwork
- Engage patients/families in care management and care coordination
- Follow-up with patients regarding missed appointments
- Home visits to assess environments to help improve health outcomes
- Mammogram referral coordination
- Show Me Healthy Women/WISEWOMAN Coordination
- Asthma and Diabetes Education
