For complete anonymity, please submit your report from a smart phone or a computer that is not on MOCH’s network, such as a computer located at home or at a public library. If you would like to leave your name and/or contact information in your submission, you can feel free to do so; however, it is not required.
Please be as specific as possible in your report so that the matter may be investigated to the fullest extent. Once your report is received, the MOCH Compliance Officer will investigate the matter and provide corrective action, if necessary.
