Dental care is offered at all six clinic locations; specializing in preventative and restorative dental care. A wide variety of dental insurance is accepted as well as all managed Medicaid plans, and the sliding fee program is available to all who qualify.
Missouri Ozarks Community Health Dental Providers
Dr. Sybil Fortner is a General Dentist and has served as the Dental Director for Missouri Ozarks Community Health since 2007. She earned her undergraduate degree from the University of Louisville and her Doctor of Dental Surgery degree from UMKC School of Dentistry.
Dr. Fortner spends majority of her time providing care to rural school districts on the mobile dental bus. When not providing care on the dental bus, she sees patients in the Ava Clinic

Dr. Durbin graduated from the UMKC School of Dentistry in 2014 and has enjoyed an active practice in local community health since that time. Joining the Missouri Ozarks Dental team in 2016, he is enthusiastic about bringing quality care and a personal touch to our patients. Dr. Durbin is a member of the Board of Directors for the Missouri Academy of General Dentists as well as the Greater Springfield Dental Society. He is a native of the Springfield area and enjoys outdoor recreation and gardening, his cup of coffee, a good read and time with family and friends.
Dr. Durbin sees patients in the Mansfield office Monday, Wednesday and Thursday. He also provides urgent dental care in the Cabool clinic every Tuesday.
Dr. Bales graduated from the UMKC School of Dentistry in December of 2002. Dr. Bales has over 13 years experience in family dentistry and loves working with children. She is originally from Missouri and enjoys spending time with her husband and children.
Dr. Bales is available to see patients Monday-Thursday in the Mountain Grove clinic.
Cody Thress graduated from UMKC School of Dentistry in May 2014 and has enjoyed an active practice in Community Health since graduation. He joined Missouri Ozarks Dental team in April 2018, and is looking forward to serving the community in Houston, MO. He is a native of Branson, MO and enjoys spending time with family and time outdoors.
Dr. Thress is available to see patients Tuesday–Friday in the Houston clinic.
Originally from the east coast, Dr. Casimir first came to SW Missouri for college in pursuit of a degree in biology. He attended and graduated UMKC School of Dentistry in 2014, and returned to this part of the state with a passion for helping those who have limited resources. He previously practiced at Jordan Valley Community Health Center from 2014 to 2018 as the dental provider at their satellite clinic in Republic, providing care at a high standard for the underserved, and now provides that same level of care here at MOCH.
He currently lives in Springfield with his wife and 3 year old daughter, and enjoys physical fitness, reading, gardening, and coming up with creative home improvement projects.
Dr. Casimir is available to see patients every Monday, Tuesday, Wednesday and Friday in the Ava clinic.
Kammi graduated with an AS degree in Dental Hygiene from Wichita State University in 1984. She has held licensure in Missouri, Kansas, Oklahoma, Indiana and Kentucky. She practiced dental hygiene in private practice settings for 22 years and public health for the past 7 years with Missouri Ozarks Community Health. She is actively involved as a board member for MO Dental Hygiene Association and as a local component president since 2010 for the Greater Springfield Dental Hygiene Association. Charlee graduated with an Associate of Applied Science degree in Dental Hygiene from Ozarks Technical Community College in 2015. Before going to dental hygiene school she worked for 2 years as a dental assistant in Springfield, Missouri but is originally from Plato, Missouri and is now glad to be providing care in a community close to home. She loves to spend time with her husband. They enjoy spending time outdoors; camping, floating, fishing, hunting, and biking.
