MOCH is a provider for many payment types including:

- Various Other Insurance Types
If you currently have insurance coverage through a company that we are not providers for, we will work with your coverage type to enroll as an approved provider.
MOCH offers a “sliding fee program” for any individual. Clients may qualify for this program if income guidelines are met and proof of income is presented. If qualifications are met, service charges will be reduced.

Dr. Sherard joined Missouri Ozarks Community Health November 2011.
She is from Ava and is serving as a General Family Practice physician. She is available to see patients at the Ava Clinic, Monday–Wednesday, 8:30am–5:30pm.

Kathy has been with Missouri Ozarks Community Health since 2007.
She is a Certified Family Nurse Practitioner, providing a wide variety of patient care. Previous to becoming a Nurse Practitioner, Kathy was an Registered Nurse at Mercy Hopsital, providing care in the trauma center.
Kathy is available to see patients in the Mansfield clinic, Monday-Thursday, 7am-5pm.

Ida graduated from UMKC with her Masters in Nursing in 1998 as has over 18 years experience practicing as a Family Nurse Practitioner.
She graduated from Nursing School from Westark Community College in Fort Smith, Ark. in 1976.
Ida also served in the U.S. Army from 1986 to 2006 and retired as a Major.
Ida is available to see patients in the Ava clinic, Monday-Friday, 7am–3:30pm.
Charlene Furr has been a primary care nurse practitioner for nearly 18 years and dearly loves providing care and support to her family practice clients.
She graduated as a nurse from Kansas City’s Research Medical Center and received her Masters in Nursing at the University of South Carolina in the FNP tract.
She is a huge advocate for her patients and works hard to improve their overall health outcomes. In her free time, she enjoys quilting, camping and enjoying the National Parks! She is very excited to return to her farm in Solo, Missouri and provide high quality patient care in the Houston clinic and surrounding Missouri Ozarks clinics.
Charlene is available to see patients in the Houston clinic, Monday-Friday, 8am-5pm.
Mina Higgins FNP graduated from Missouri State in 2000.
She has over 20 years’ experience as an FNP in many rural areas of Missouri. She is happy to serve the health care needs of Ozark County, MO!
Mina is available in the Gainesville clinic, Monday-Friday, 8am-5pm.
Dr. Ball is a 1980 graduate of the Kirksville School of Osteopathic Medicine in Kirksville, MO. and is Board-Certified in General Practice.
Dr. Ball has been providing family practice medicine since 1981 in Mountain Grove, MO.
Dr. Ball is available to see patients every Wednesday, 8am-5pm in the Cabool clinic.
Dr. Wolfe and his family moved to Texas County from Colorado in 2013 after finishing his medical training in family medicine.
He has enjoyed serving the local community as he raises his five boys ages 9 to 2 with his wife, Melissa. They look forward to continuing to provide a broad spectrum of quality care in this very special part of the country.
Dr. Wolfe is available to see patients in the Houston clinic, Monday-Friday.
Dr. Augusta Hays is a board certified family physician that attended medical school and residency at the University of Missouri in Columbia.
She worked as a Commissioned Officer in the United States Public Health Service with the Indian Health Service for over 20 years. In this capacity she worked as a family physician on reservations from Arizona to Alaska. She has subsequently worked in urban and rural community health centers that help bring care to areas where it is needed.
Dr. Hays has additional training in preventive medicine and public health.
Dr. Hays is available to see patients in The Mansfield clinic on Mondays and Tuesdays and in the Ava Clinic on Thursdays and Fridays.
Brenda graduated from Graceland University with her Masters of Science in Nursing (MSN) in May of 2014 and is a certified Family Nurse Practitioner (FNP).
She attended Southwest Missouri State University in West Plains, MO graduating with her Associates Degree in Nursing (ADN) in 1997. Brenda worked as a Registered Nurse (RN) in a variety of disciplines including Medical Surgical, Operating Room, Intensive Care, Long Term Care, Emergency Room, and Travel Nursing which has given her a diverse knowledge base for primary care. She provides medical care to children and adults of all ages for both wellness visits and acute illnesses or injuries. Brenda was raised in Cabool, MO and is pleased to provide medical services in her home town and surrounding communities.
Brenda is available to see patients in the Cabool clinic, Monday-Friday, 8am-5pm.
Jacqueline Austin originally grew up in Ava, MO., and moved to California in 1995. She has spent over 12 years providing care as a Registered Nurse in both the emergency and urgent care settings, as well as the perioperative setting.
Jacqueline completed her student rotation at Missouri Ozarks Community Health and graduated in December 2018 as a Family Nurse Practitioner.
She is very excited to return to the communities she grew up in and provide high quality care to MOCH patients. In her free time, she enjoys spending time with family and her 3 dogs. Jacqueline will be working in the Mansfield clinic, Monday-Friday, 11am-7pm.
Natalie Jeffery is a Family Nurse Practitioner who began practicing at Missouri Ozarks Community Health in August, 2019.
Natalie started her career in the medical field in 2004, beginning with her Associates Degree and completing her MSN-FP program in 2018. Natalie and her husband have 2 daughters and in her spare time, she enjoys reading, running and spending time with her family and dogs.
Natalie is available to see patients in the Licking clinic, Monday-Friday, 7:30am–4:00pm.
Kalynn graduated from Missouri State University in December 2019 with her Masters degree in Physician Assistant Studies. Prior to that, she attended Evangel University and obtained her Bachelors degree in Biological Chemistry.
She is from Ava, MO and grew up on the family farm. She loves the people that call the Ozarks their home and looks forward to serving them at the Mountain Grove clinic. She and her husband enjoy serving in their church, hanging out with their family, friends, and dogs, and spending as much time as possible outside and on the farm.
Kalynn is available to see patients in the Mountain Grove clinic Monday-Friday, 8:00am-5:00pm.
