MOCH is a provider for many payment types including:

If you currently have insurance coverage through a company that we are not providers for, we will work with your coverage type to enroll as an approved provider.
MOCH offers a “sliding fee program” for any individual. Clients may qualify for this program if income guidelines are met and proof of income is presented. If qualifications are met, service charges will be reduced.

Dr. Sherard joined Missouri Ozarks Community Health November 2011.
She is from Ava and is serving as a General Family Practice physician. She is available to see patients at the Ava Clinic, Monday – Wednesday, 8:30am – 5:30pm.

Kathy has been with Missouri Ozarks Community Health since 2007.
She is a Certified Family Nurse Practitioner, providing a wide variety of patient care. Previous to becoming a Nurse Practitioner, Kathy was an Registered Nurse at Mercy Hopsital, providing care in the trauma center. Kathy is available to see patients in the Ava clinic, Monday, Tuesday, Thursday and Friday 8:30am – 5:30pm, and in Mansfield on Wednesdays, 8:30am – 5:30pm.

Ida graduated from UMKC with her Masters in Nursing in 1998 as has over 18 years experience practicing as a Family Nurse Practitioner.
She graduated from Nursing School from Westark Community College in Fort Smith, Ark. in 1976.
Ida also served in the U.S. Army from 1986 to 2006 and retired as a Major.
Charlene Furr has been a primary care nurse practitioner for nearly 18 years and dearly loves providing care and support to her family practice clients.
She graduated as a nurse from Kansas City’s Research Medical Center and received her Masters in Nursing at the University of South Carolina in the FNP tract.
She is a huge advocate for her patients and works hard to improve their overall health outcomes. In her free time, she enjoys quilting, camping and enjoying the National Parks! She is very excited to return to her farm in Solo, Missouri and provide high quality patient care in the Houston clinic and surrounding Missouri Ozarks clinics.
Connie graduated from Graceland University in 2005 with a Masters degree in Nursing/Family Nurse Practitioner (FNP).
Connie has over 10 years experience as a FNP. She previously worked as an EMT/Paramedic, both on the ambulance and in the emergency department as well as being a Flight Paramedic.
Dr. Ball is a 1980 graduate of the Kirksville School of Osteopathic Medicine in Kirksville, MO. and is Board-Certified in General Practice.
Dr. Ball has been providing family practice medicine since 1981 in Mountain Grove, MO.
Missouri Ozarks Community Health announces the addition of Dr. Selma Taylor, OB-GYN to our staff. Dr. Taylor is board certified in obstetrics and gynecology, specializing in breast and cervical cancer screening, as well as prenatal care, contraceptive management, and pre-conception planning.
Dr. Taylor attended medical school at Kansas City University of Medicine and Biosciences, practicing for the last seven years in Southern Missouri. Dr. Taylor serves patients ranging from the very young to those in their later years. Dr. Taylor will be available in all six locations of Missouri Ozarks Community Health, including Ava, Cabool, Gainesville, Houston, and Mountain Grove, with prenatal visits available in the Mansfield Clinic.
Dr. Taylor enjoys spending time together with her husband taking walks, fishing, traveling and watching movies.
Dr. Wolfe and his family moved to Texas County from Colorado in 2013 after finishing his medical training in family medicine.
He has enjoyed serving the local community as he raises his five boys ages 9 to 2 with his wife, Melissa. They look forward to continuing to provide a broad spectrum of quality care in this very special part of the country.
Katie graduated from Missouri State University with her Doctor of Nursing Practice (DNP) in May 2017 and is a certified Family Nurse Practitioner (FNP).
She previously graduated from Central Methodist University in Fayette, MO in 2013 with her Bachelor of Science in Nursing (BSN). She has worked as a Registered Nurse (RN) in the Emergency Department since 2013. Katie is originally from a small, rural town in northern Missouri and enjoys being able to give back to the rural community through providing primary care.
Brenda graduated from Graceland University with her Masters of Science in Nursing (MSN) in May of 2014 and is a certified Family Nurse Practitioner (FNP).
She attended Southwest Missouri State University in West Plains, MO graduating with her Associates Degree in Nursing (ADN) in 1997. Brenda worked as a Registered Nurse (RN) in a variety of disciplines including Medical Surgical, Operating Room, Intensive Care, Long Term Care, Emergency Room, and Travel Nursing which has given her a diverse knowledge base for primary care. She provides medical care to children and adults of all ages for both wellness visits and acute illnesses or injuries. Brenda was raised in Cabool, MO and is pleased to provide medical services in her home town and surrounding communities.
Erin Carlile, FNP graduated from Walden University with Masters of Science in Nursing in 2014 and is a certified Family Nurse Practitioner. She graduated from Baker University in Topeka, KS in 2007 with Bachelor of Science in Nursing (BSN).
Erin worked as a Registered Nurse in the Operating Room and in Critical Care Unit while living in Kansas. Erin and her family have recently moved to Missouri from Kansas where she worked in a rural health clinic. She is excited to continue serving in that capacity in the Gainesville area.
Jessica graduated from Cox College with her Master of Science in Nursing (MSN) and is a certified Family Nurse Practitioner (FNP). She has worked in the nursing field for 17 years. Previous to becoming a Nurse Practitioner, Jessica was a Registered Nurse (RN) providing care in a variety of disciplines including School Nurse, Medical Surgical Nurse, Nurse Educator, Care Coordinator, and Nursing Management.
Jessica grew up in Fordland, MO where she and her husband and children still reside. She is excited to join the MOCH team and provide care to local rural communities. Jessica is available to see patients in the Ava clinic, Monday – Friday 8:00am – 5:00pm.
Jacqueline Austin originally grew up in Ava, MO., and moved to California in 1995. She has spent over 12 years providing care as a Registered Nurse in both the emergency and urgent care settings, as well as the perioperative setting.
Jacqueline completed her student rotation at Missouri Ozarks Community Health and graduated in December 2018 as a Family Nurse Practitioner.
She is very excited to return to the communities she grew up in and provide high quality care to MOCH patients. In her free time, she enjoys spending time with family and her 3 dogs. Jacqueline will be working in the Mansfield clinic, 11am -7pm Monday-Friday.
