New Patients

What to Bring

On your first visit, please plan to spend the first 30 minutes completing registration forms. Or you can complete the forms prior to your visit. They are provided on our website for your convenience. Print them out and bring them with you. It’s also important to bring the following forms of identification with you:

  • Completed forms
  • Photo ID
  • Social Security Card
  • Insurance Provider Card (s)
  • A list (or the bottles) of all your prescriptions
  • Immunization records

Common Questions

How early should I arrive for my appointment?

A new patient should plan to arrive 30 minutes before their appointment time so that paperwork may be filled out and processed for the doctor or nurse prior to the appointment. Returning patients should plan to arrive 15 minutes before their appointment to check-in.

Who can help me find other resources?

Our Patient Advocates are available during normal business hours, Monday – Friday, 8 a.m. to 5 pm. to help patients in all of our locations. Care Coordinators work with our patients’ health care team to connect patients to the best resources available.

What’s the best way to talk with my provider?

If you’re wondering how to ask your provider about a new medication you’re taking or when you should come in to have your blood pressure checked, etc. we suggest using our Patient Portal.

Does Missouri Ozarks Community Health offer a Sliding Fee Program?
